For International Medical Graduates (IMGs) planning to apply for the U.S. residency match, one of the most important factors in your application is the U.S. Clinical Experience (USCE). The timing of your USCE rotations can play a crucial role in strengthening your application, improving your Letters of Recommendation (LoRs), and increasing your chances of securing interviews with top residency programs.

1. IMGs Should Start Your USCE as Early as Possible if Applying for The Residency Match
Ideally, IMGs should aim to start USCE as soon as possible in order to build a strong foundation of experience and secure high-quality LoRs in time for the residency match. This not only provides the opportunity to work with U.S. physicians but also gives you the chance to develop essential clinical skills that will be recognized by U.S. residency programs.
When to start: Ideally, you should plan to begin your clinical rotations at least 12 to 18 months before the match. So, if you're applying for the September match in year X, your first rotation should begin in early to mid-year X-1 (12 months prior to the match). Starting early will give you ample time to gather LoRs, make connections with attending physicians, and improve your clinical skills.
2. Scheduling USCE in the Year of the Match
If you want to make sure your USCE is fresh and relevant for the September match, the ideal time to complete USCE in the year of the match would be from January to June. This gives you the opportunity to:
Enhance your clinical skills: By engaging in hands-on clinical experience just before the match, you’ll ensure that your clinical skills are sharp and up-to-date, which is crucial when applying to competitive programs.
Collect Strong LoRs: Completing rotations early in the year ensures that the attending physicians have enough time to observe your skills and write strong Letters of Recommendation for your residency application. These LoRs should ideally be submitted by July or August to meet the residency application deadlines.
Boost Your Application with Recent Experience: Residency programs often prefer candidates who have recent U.S. clinical experience, as it reflects your ability to work within the U.S. healthcare system and interact with patients in the U.S. setting. Completing rotations in the year of the match (specifically from January to June) will demonstrate that you are familiar with current U.S. practices.

While as an IMG it’s important to get as much usce as possible for the residency match, the quality of your rotations and the strength of your LoRs are far more important than the sheer number of rotations you complete. Focus on securing rotations in well-regarded hospitals and clinics where you will have opportunities to interact with experienced attending physicians who can write impactful LoRs.
Tip: Aim for rotations in your chosen specialty (or specialties that align with your interests), as residency programs want to see that you’ve gained experience in your intended field.

4. Final Push: Short-Term USCE Before Application Submission
While IMGs should aim the bulk of their USCE should ideally be completed earlier in time for the residency match application deadline, it’s not uncommon for IMGs to complete additional rotations or observerships right before submitting their residency applications (between July and August).
